Overview

In this recorded Cardiology Grand Rounds presentation, Professor of Medicine/Cardiology and Heart Failure Section Chief at the Medical University of South Carolina (MUSC) Dr. Ryan Tedford discusses the right ventricle (RV). Dr. Tedford begins with reviewing past research into the right ventricle and its important in right sided heart function/contribution to mortality and morbidity in many heart-related conditions. He then moves on to describe how to assess the right ventricle, reviewing the 4 main determinants of RV function and how dysfunction of any of these 4 areas can start a vicious cycle within the heart. Dr. Tedford then describes RV afterload sensitivity, pressure volume loop, contractility, and preload reduction. Dr. Tedford then delves into pressure volume loops in greater detail as well as reviews research and data regarding RV ejection fraction (EF). Shifting gears, Dr. Tedford then discusses RV contractility in depth, touching first on cardiac anatomy and changes after cardiac surgery. He then moves on discuss how to manage high risk patients and the learning curves associated with less invasive strategies for those suffering from RV failure. Dr Tedford concludes his lecture by asking attendees to consider mechanistic determinants of right heart function, how assessment of the RV is complicated by afterload sensitivity, preload optimization, afterload reduction, increased contractility and rhythm control, and/or support devices.
