Overview
In this recorded Anesthesia Grand Rounds presentation, VP and Chief Quality Officer at UT Southwestern Medical Center Dr. William C. Daniel discusses UT Southwestern Medical Centers journey towards organizational excellence and high reliability. Dr. Daniel outlines UT Southwestern’ s strengths in research and growth, but acknowledges weaknesses in areas like psychological safety and hierarchical culture. He shares their alignment process through an operational plan, balanced scorecard, and celebration of excellence. Key results highlighted include improvements in heart failure readmissions and reductions in mortality rates. However, the Dr. Daniel emphasizes that safety remains a challenge due to hesitancy of providers to speak out. He introduces their use of an OR black box to create a psychologically safe space for team learning and cultural transformation. The presentation culminates with the decision to embark on a high reliability journey, leveraging the learnings from the OR black box initiative.

Objectives
At the conclusion of this activity, learners should be able to:

  1. Discuss how to socialize the OR black box initiative across organization, involving stakeholders and addressing concerns to build trust and acceptance.
  2. Implement enhanced M&M (morbidity and mortality) sessions, using video clips from the OR black box to facilitate open discussions and team learning.
  3. Establish governance structures and processes to support the OR black box initiative, ensuring transparency, team learning, and a non-punitive approach.
  4. Develop policies and procedures to address issues identified through the OR black box, such as interruptions during counts and the use of proper names among team members.
