Overview
In this recorded Palliative Care Educational Series presentation, palliative care and addiction medicine specialist Dr. Janet Ho provides a comprehensive overview on the use of buprenorphine, a partial opioid agonist, for pain management and treatment of opioid use disorder. Dr. Ho begins with an overview of buprenorphine's pharmacology, including its unique properties as a partial agonist with a strong binding affinity to the mu-opioid receptor, its ceiling effect on respiratory depression, and its ability to reduce hyperalgesia and cravings. Dr. Ho also discusses different formulations of buprenorphine in detail, including transdermal patches, buccal films, sublingual tablets, and injectable depot formulations. The appropriate dosing, titration, and administration of each formulation are explained, along with cost considerations. She also covers strategies for managing acute pain in patients already taking buprenorphine, such as splitting doses or adding a full opioid agonist like morphine or hydromorphone. Case studies are presented to illustrate the practical application of buprenorphine in various clinical scenarios.
