In this recorded Anesthesia Grand Rounds presentation, Anesthesiologist Dr. Yun-Yun Kathy Chen, of Brigham & Women’s Hospital, discusses regional anesthesia for patients at risk of compartment syndrome and severe post-operative pain. Dr. Chen begins with a historical review of regional anesthesia and compartment syndrome, current evidence on nerve blocks masking compartment syndrome symptoms, quantitative sensory testing showing partial nerve blocks reduce but don't eliminate ischemic pain, individual psychosocial factors, and personalizing nerve block procedures with sedation versus education/reassurance. She then analyzes research studies and clinical recommendations to demonstrate approaches balancing sufficient analgesia and ability to detect compartment syndrome. Dr. Chen concludes by evaluating careful use of low-dose regional anesthesia and multimodal pain management for high-risk orthopedic trauma patients.
