SHC Medicine Care Review Committee

The Internal Medicine Peer Review Committee is a recurring multidisciplinary educational forum focused on improving the quality, safety, and reliability of care for hospitalized adult patients with complex medical conditions. The conference reviews cases selected through mortality review, unplanned readmissions, unexpected clinical deterioration, diagnostic or management challenges, and other quality concerns to identify opportunities for improvement. Participants examine clinical decision-making, care transitions, interdisciplinary communication, and systems factors to apply evidence-based practices, reduce unwarranted variation, and improve patient outcomes across the continuum of internal medicine care.

AMA PRA Category 1 Credits™ (1.50 hours), Non-Physician Participation Credit (1.50 hours)

1. Apply evidence-based guidelines and best practices to improve diagnostic reasoning, clinical decision-making, and management of medically complex patients.
2. Analyze peer review cases to identify opportunities to improve patient safety, timely escalation, communication, care coordination, and systems-based care.
3. Implement lessons learned from peer review and collaborate with the interprofessional healthcare team to reduce unwarranted practice variation and improve the quality, safety, and reliability of internal medicine care.

In support of improving patient care, Stanford Medicine is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team. 
 
Credit Designation 
American Medical Association (AMA) 
Stanford Medicine designates this Live Activity for a maximum of 1.50 AMA PRA Category 1 Credits^TM.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.  

American Board of Internal Medicine MOC Credit 
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1.50 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
