In this recorded Neurology Grand Rounds presentation, Dr. Greg Albers discusses expanding the indications for mechanical thrombectomy. In his lecture, Dr. Greg Albers delivers a comprehensive presentation on recent developments in thrombectomy trials for stroke treatment. He reviews 16 trials that came in four waves, each with significant findings that have shaped stroke treatment practices. The first wave, dubbed 'Wipe Out', revealed in 2013 that a decade-old treatment was ineffective. The second wave in 2015 showed thrombectomy's effectiveness within 6 hours for patients meeting specific criteria. The third wave in 2018 extended the treatment window to 24 hours for patients with significant penumbra. The current fourth wave focuses on treating patients with large core infarcts. Dr. Albers discusses the results of six recent trials investigating the efficacy of thrombectomy in patients with large core infarcts, including RESCUE Japan LIMIT, TENSION, TESLA, ANGEL-ASPECT, and SELECT2. He emphasizes the importance of patient selection, imaging techniques, and the challenges in determining the true extent of irreversibly damaged brain tissue. The presentation highlights the evolving understanding of what constitutes 'dead' brain tissue and the potential for salvaging seemingly damaged areas. Dr. Albers also touches on the future of neuroprotective strategies and the role of AI in improving stroke treatment decisions. The discussion concludes with considerations for updating treatment guidelines based on these new findings.
