Objectives
At the conclusion of this activity, learners should be able to:

  1. Consider using topical over-the-counter hydrocortisone cream or fluticasone spray to manage skin reactions associated with buprenorphine transdermal patches.
  2. For patients transitioning to hospice care, communicate with the hospice team regarding their comfort level and experience with managing buprenorphine treatment.
  3. When tapering patients off opioids, consider rotating them to buprenorphine as a safer alternative, especially if they exhibit complex persistent opioid dependence or have difficulty tapering off the full agonist opioid.
  4. For patients with opioid use disorder, aim for a buprenorphine dose of at least 16 mg, and preferably 24 mg, before reducing or discontinuing the full agonist opioid.
  5. For patients without opioid use disorder, consider a buprenorphine dose between 6-12 mg as a potential target, adjusting based on pain control and functional improvement.
  6. Develop a detailed treatment plan outlining the buprenorphine initiation and titration schedule, as well as the plan for managing the full agonist opioid and any short-acting opioids for breakthrough pain.
