Overview
Project ECHO Diabetes offers (1) weekly tele-education clinics with continuing education credits for primary care providers (PCPs) and pharmacists treating patients with diabetes on intensive insulin therapy, (2) support with complex diabetes-related medical decision-making, and (3) access to peer mentors for patients with diabetes. Project ECHO Diabetes addresses urgent geographic, racial, and socioeconomic disparities to promote health equity and improve health outcomes in California's highly vulnerable populations of children and adults with diabetes. The goal of the program is to democratize specialty knowledge by increasing the capacity of PCPs, pharmacists, and clinics to empower and safely and effectively manage underserved patients with diabetes who do not receive routine specialty care. Stanford University will serve as the “Hub” site with a team of multi-specialty experts (pediatric and adult endocrinologists, primary care providers, behavioral health specialists, nurse/diabetes educators, social workers, dietitians, and exercise physiologists) to work in partnership with “Spoke” sites (community healthcare providers and clinics).

Objectives
At the conclusion of this activity, learners should be able to:

  1. Understand what diabetes stigma is, what it looks like, and what the impacts of stigma are on mental and physical health
  2. Increase awareness of specific ways in which stigma persists in the healthcare space
  3. Become familiar with resources such as the dStigmatize.org website that can provide continued learning, as well as guidance for reducing stigma.

Accreditation

In support of improving patient care, Stanford Medicine is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team. 

Credit Designation 
American Medical Association (AMA) 
Stanford Medicine designates this Live Activity for a maximum of 1.00 AMA PRA Category 1 CreditsTM.  Physicians should claim only the credit commensurate with the extent of their participation in the activity. 

Accreditation Council of Pharmacy Education (ACPE)

American Board of Internal Medicine MOC Credit 
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1.00 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Go to Stanford's CME portal > My CE > Profile tile > Credentials > Select NABP ePID# from the drop down and enter your ID number.
Please also enter your Date of Birth (month and day) in your profile under Basic Information.  The field will appear when you select your Profession as Pharmacist.

MOC Participation Threshold

Step 1: Learner participates in verbal or chat-based case discussion/conversation as judged by a group leader or observer.
Step 2: Learner completes the session evaluation within one week of the session date.
Step 3: Learner claims credit for the session in Stanford’s CME portal here. 
Step 4: Learner completes the MOC section in Stanford’s CME portal if s/he wishes to claim MOC credit. Go to My CE > Profile tab > go to MOC section and enter Date of Birth (month and day).  Please also provide permission for your MOC information to be shared and transmitted to ACCME and provide your Board Diplomate ID number.
